Information for Consultants and Clinics 

In order to qualify for the Positive Health programme patients must meet the following Referral Criteria:

• Aged over 18
• HIV positive
• Currently have an inactive lifestyle or are suffering from a HIV related condition that would be improved by regular and supported physical activity 
• Prepared to commit to 3 sessions a week for a 12 week period 

How to refer a patient onto the Positive Health scheme: 

• Ensure the individual meets the above Referral Criteria 
• Download and complete the Referral Form 
• Hand Referral Form to your patient for them to submit to the Club’s Positive Health Coordinator. Alternatively, the completed Referral Form can be posted to the address detailed on the Form. Please note, the form cannot be faxed due to confidentiality.  

Patients can be referred onto the Positive Health programme at any time. We only accept referrals from registered HIV agencies and other specialist HIV organisations.  To find out whether your organisation is currently listed with us, check our Referring Hospital List.
